From Aisha (ra): The Prophet (pbuh) would pray for the sick as follows:

“Remove this illness, O Lord of mankind! Grant healing, for You are the Healer. There is no healing except that which You provide. Grant such healing that no illness remains.” (Bukhari, Marda, 20; Muslim, Salam, 46)
